Information we collect
We collect information you choose to submit through our membership interest, contact, and provincial survey forms. This may include your name, email address, phone number, organization, role, region, survey responses, and written comments.
Website analytics
We record the pages visited, the date and time of each visit, and a randomly generated browser-session identifier so APCHNL can understand general website use. The tracker does not record your name, email address, IP address, or form responses, and the session identifier is not used to identify you.
How information is used
Information may be used to respond to inquiries, provide initiative or membership updates, understand sector priorities, plan resources and training, support aggregate analysis, and develop APCHNL’s advocacy work.
Provincial survey
Survey contact information is optional. Survey findings may be summarized and shared in aggregate. APCHNL will not intentionally publish a respondent’s name, email address, or personal care home without permission.
Resident and health information
Our public forms are not intended for resident records, complaints containing personal health information, or confidential resident details. Please do not submit resident names, identifying information, or personal health information through this website.
Sharing and disclosure
We do not sell personal information. Information may be disclosed where required by law or to service providers supporting the secure operation of the website, subject to appropriate safeguards.
Retention and security
Information is retained only as reasonably needed for the purposes described above. Reasonable administrative and technical measures are used to protect submitted information; however, no online service can guarantee absolute security.
Your choices
You may ask to review, correct, or delete personal information you have submitted, subject to legal and operational requirements. You may also ask not to receive future initiative communications.
